Conclusiones: Fracturas Intertrocántericas
Consideramos que este simple método, fácilmente reproducible, es de gran utilidad para describir la localización del implante. Si en forma rutinaria realizamos estas mediciones nuestra curva de aprendizaje será rápida y el cirujano cuantificando la distancia punta-apex (TAD) podrá tener un pronóstico sobre la posibilidad de extrusión del implante o de un mal resultado.
La calidad de las radiografías desde el ingreso del paciente hasta la consolidación de la fractura son importantes, en ocasiones hay dificultad con la proyección lateral.
La fractura inestable es muy exigente, se debe buscar una reducción lo más anatómica posible; y el implante debe quedar en una verdadera posición central en las dos proyecciones.
Debemos buscar alternativas que nos permitan intervenir nuestros pacientes en forma más precoz, y buscar una mejor estabilidad en las fracturas inestables, para disminuir las complicaciones.
Nos permitimos sugerir este método de evaluación de la reducción y la posición del implante como parte del seguimiento de las fracturas intertrocántericas de la cadera.
