Variables a Vigilar en Pacientes en Ventilación Mecánica por Asma

El objetivo de los esquemas de ventilación propuestos es lograr un adecuado grado de oxigenación del paciente manteniendo niveles seguros de hiperinflacion dinámica lo que, como consecuencia, generara hipercapnia y acidosis respiratoria en un alto porcentaje de pacientes.

Algunos elementos para su vigilancia son:

  1. Presion inspiratoria pico: Algunos autores recomiendan mantener la presión inspiratoria pico en valores por debajo de 50-60 cmH2041. Otros autores creen que se pueden permitir valores más altos ya que la mayor parte de esta presión es transmitida a la vía aérea y no al alvéolo43.
  2. Presion Plateau: esta presión aumenta en forma paralela con la hiperinflacion dinámica en concordancia con la adaptabilidad pulmonar, lo que la convierte en un parámetro más valioso para la monitoría de estos pacientes 34.
  3. Tension arterial, presión venosa central y presión esofágica: Estos tres factores están influidos directamente por la excesiva hiperinflacion dinámica. Su medición debe hacerse de manera dinámica y posterior a un período de apnea una caída en la PVC o la presión esofágica y/o un aumento en la tensión arterial indican una excesiva hiperinflacion dinámica.
  4. Hipoventilación: No hay una regla absoluta para su instauración; el objetivo es evitar o disminuir al máximo la hiperinflacion dinámica. Esto permitirá ir aumentando la ventilación a medida que se controle la obstrucción de la vía aérea41.

¿ Cuándo Retirar la Ventilación Mecánica?

El principal problema de estos pacientes es el imbalance entre la capacidad ventilatoria y los requerimientos ventilatorios. Independientemente de la técnica de extubación que se utilice (revisión que se sale de los objetivos de este documento), se debe tener en cuenta que los requerimientos ventilatorios dependen de la carga de CO2 que debe ser excretada, la eficiencia de los pulmones para excretar el CO2 (VD/VT) y el nivel de PCO2 buscado.

El mejor momento para modificar el esquema ventilatorio es aquel en el que se ha logrado un control de la obstrucción y de la hiperinflacion, señalado por el cambio en la presión Plateau y en el Autopeep. Si no hay complicaciones de la enfermedad o del tratamiento ventilatorio el proceso de extubación es usualmente rápido y el éxito de los métodos utilizados no parece ser diferente con respecto a pacientes no asmáticos44.
