Enfermedad Hemorroidal, Discusión

El tratamiento de la enfermedad hemorroidal ha suscitado controversia a través de los años a pesar del progreso en la técnica quirúrgica. La razón de esto se encuentra en los buenos resultados informados por los diferentes autores con sus técnicas preferidas; sin embargo, en la actualidad todo parece conducir al uso de las bandas de goma para las hemorroides internas grado I y 11Y hemorroidectomía cerrada tipo Fergusson para las grado 1II y IV Y para las externas ( 1, 2, 12-21, 27).

Aunque hay trabajos que respaldan el empleo de la crioterapia y escleroterapia, comienzan ya a encontrarse pacientes con complicaciones locales como abscesos, oleomas. dolor y estenosis, siendo esta última de difícil tratamiento (L 2. 7-1 L 22-26).

La fotocoagulación infrarroja y la diatermia bipolar, también tuvieron su momento como procedimiento de elección en el tratamiento de la enfermedad hemorroidal. ya que al compararla específicamente con la ligadura con bandas de goma, los resultados iniciales eran similares, pero últimamente también han caído en desuso (28-30. 32). El láser ha demostrado ventajas sobre los demás métodos de tratamiento de esta patología, pero su costo y el abuso que se hace de él lo ha dejado, por ahora. en un segundo plano dentro de los cirujanos dedicados al tratamiento de la patología anorrectal (28. 30).

Es por ello que el grupo de cirugía general de la Clínica San Rafael de Santafé de Bogotá, propone la hemorroidectomía tipo Fergusson para el tratamiento de la enfermedad hemorroidal. basándose en los resultados obtenidos en un estudio prospectivo de 10 años y apoyado por los informes de la gran mayoría de autores e incluso de la Sociedad Americana de Coloproctología.

Los pacientes con diagnóstico de enfermedad hemorroidal, si no son sometidos a tratamiento médico riguroso a base de dieta y sustancias formadoras de bolo fecal, a largo plazo pasan del grado I al IV, y cuando continúan sintomáticos a pesar de seguir un tratamiento juicioso, sin importar el grado de su enfermedad hemorroidal, se considera que deben ser llevados a cirugía.

El colon por enema es importante como estudio de extensión en pacientes mayores de 40 años por la probabilidad de presentar patologías asociadas, o en menores de 40 años con histoira sugestiva de otra patología del colon. En el presente estudio la utilidad práctica fue sólo para el 10% de los pacientes estudiados.

Abstract

A prospective study to evaluate the surgical treatment of hemorrhoids employing the Fergusson’s technique. was performed at the San Rafael Clinic of Bogotá between January 1983 and January 1991.

One hundred and thirty-two medical records were reviewed. finding an average age (Jf 40 years. Forty-eight percent of the patients were male and 52% female; of them, 70 patients (53%) had mixed hemorrhoids, 51 (39%) internal and 11 (8%) external.

Rectal bleeding was found to be the chief complaint in 34 (26%) of the patients while 66 (50%) complained of constipation.

Proctosigmoidoscopy was performed in al! the patients as the main work-up procedure. Barium enema was petformed (mly on 40% of the patients.

Alto 8 yearfol!ow-up was observed with an average Of 5 years, which revealed complications following the procedure in (mly 4% (Jf the patients. Results obtained are discussed in accordance with the world literature.
